Student Profile Services Team Contract

Student Profile Services expects to finish the web site design and all profiles by the requested deadline and will make every attempt to do so. If the site and stories are not completed within seven days after the requested deadline, the group representative may decline the service and receive a full refund of all advance payments.

The group representative (one person only) agrees to pay Student Profile Services for ________ profiles at the rate of $35 each for a total of $_________ (# of profiles X $35). Enclosed is 10% (# of profiles X $3.50) of the total payment in advance. The final 90% (# of profiles X $31.50) will be due immediately upon completion of the project.

Profiles may not be placed on web sites or submitted to publications until the group representative has paid the account in full. Student Profile Services makes no additional charges to any web sites or publications using the stories, but a line crediting www.studentprofileservices.com must appear with the profiles.

The team or activity web site using the set of profiles will display at least one hyperlink to www.studentprofileservices.com (example: “Profiles provided by…”).

Student Profile Services will interview a coach or sponsor of the program. With reasonable cooperation, each student also will be interviewed. For everyone’s convenience, all interviews will be by phone.

If Student Profile Services receives a quality photograph (either print or digital) of the student within seven days of sign-up, the picture will be inserted into the design at no charge.

Each profile will be between 180 and 240 words. The stories will be delivered in a web site format. The group is free to integrate the profiles into its web site in ways other than how they are provided.

If a student leaves the activity, or if for any other reason profiles are not used, the same charge will apply for the work done.
